4. Application Procedures and Principles
As a data subject, you may submit your requests regarding your rights under Article 11 of the Law No. 6698 to us by filling out the Application Form for the Protection of Personal Data, which you can obtain from our website www.ottomanslifedeluxe.com or by your application that meets the minimum conditions stipulated by the Citation on the Procedures and Principles of Application to the Data Controller. As the Company, we will finalize your application free of charge as soon as possible and within thirty days at the latest, depending on the nature of your request. However, if the transaction requires an additional cost, the fee in the tariff determined by The Board will be charged by the Company.

CUSTOMER EXPLICIT CONSENT FORM PURSUANT TO THE PERSONAL DATA PROTECTION LEGISLATION
As MAT MADENCİLİK İnşaat Tekstil ve San. Tic. Ltd. Şti. – OTTOMAN’S LIFE HOTEL DELUXE, within the scope of the Personal Data Protection Law No. 6698 (“KVKK”), we request your explicit consent for the following personal data processing activities that fall outside the cases stipulated by law or required for the performance of a contract.
Within this scope;

In the event that video and/or photo recordings are taken within the hotel premises, I hereby give my explicit consent to the processing and sharing of my visual and audio data on social media platforms and/or press and media channels for the purpose of promoting the Company,

Depending on the nature of the services provided by the Company, I hereby give my explicit consent to the processing of health data belonging to myself and to my children for whom I am the legal guardian (such as allergy information, chronic diseases, etc.), as well as special categories of personal data such as blood group and religion information that may be included on identity documents, for the purposes of providing, improving, and maintaining services, and to the transfer of such data to the relevant institutions and organizations when necessary,
